Who We Are?

Health & Community Services is a registered provider under the National Disability Insurance Scheme (NDIS) operating throughout Victoria. Our mission is to empower individuals living with disabilities to achieve their lifestyle goals by providing access to a wide range of services and supports.

At Health & Community Services, we pride ourselves on our multidisciplinary team, which includes general and mental health nurses, allied health practitioners, behaviour support practitioners, social workers, and support workers. This team delivers personalised support tailored to each individual's unique circumstances, ensuring comprehensive care for all participants.

We support participants with all types of disabilities, including intellectual disabilities, physical disabilities, and mental health disabilities. Our commitment to prompt service delivery means we strive to begin providing services within 72 hours following the receipt of the required documentation and the signing of service agreements.

Our Specialisations

Mental Health and Dual Diagnosis

We specialise in mental health and dual diagnosis, addressing conditions that include a mental health problem or disorder associated with problematic alcohol and/or other drug use.

Recovery-Focused Framework

At Health & Community Services, we employ a recovery-focused framework in mental health care. This approach encompasses principles of self-determination and personalised care with an emphasis on hope, social inclusion, community participation, personal goal setting, capacity building, and self-management.

Our support workers are supervised by mental health nurses and social workers, who conduct needs-led assessments to identify goals and needs for individual participants. These assessments prescribe the types of supports to be provided and include periodic risk assessments and reassessments when needed. Our mental health clinicians liaise with other stakeholders, including public and private mental health services, general practitioners, and will initiate crisis management protocols if necessary. Our mental health nurses are also capable of administering long-acting injections and other injectable medications in collaboration with prescribing doctors.

Our Services

Physical Health/Nursing Services

Our nurses provide both long- and short-term support during the post-discharge period from the hospital. They assist with medication management, wound dressings, and more.

Supported Independent Living (SIL)

In our supported independent accommodation, we create a homely environment and practise the recovery model approach with clinicians supervising support workers. We have an after-hours clinician on call 24/7 for crisis management and resolution, who will escalate to emergency services as required.
